Seems that the URL Reputation Change Request page is broken. :( The link to -read the FAQ and the form fails on submission. Both cause 404 errors.

LOL... yeah, it works now. I received an email from one of the UI Engineers. Apparently it was a legacy page (http://www.brightcloud.com/tools/repchangerequest.php?endpoint=) left over from the redesign. It's now re-directing to the page you have listed. Thanks!
