How do I go to the form that pops up after a technician in Webroot helped me. It disappeared.

I just got off the phone with a Webroot technician.  He helped me get my webroot onto an additional device. There is a feedback form that pops up at the end of the session so that I can provide input.  But something happened and it disappeared.  I would put his name here, but I think the terms of service said I should not.  At any rate, how do I get back to that form so I can answer the questions?  I really hate to not acknowledge what a good job he did.

Are you able to look back into your history in your browser. If not I am not sure how to reach that page that support has you fill out.
