Email Question

Received an email from my nephew with a URL attached. I opened his legitimate gmail with his attachment and up comes a cartoon character of a girl. Is this something that can happen without him knowing about it, or should I ask if he is just trying to make a statement?

As a general rule I never open up an URL that is in an email even from a friend unless they have specifically talked about it in the email thread. I wouldnt open the URL again and I would ask your nephew about it.
